About The Position

Under the direction and supervision of a licensed nurse, performs basic nursing care procedures. Implement a variety of recreation programs and services to residents that are socially, physically and cognitively stimulating.

Requirements

  • High School diploma or GED preferred
  • 6 months – 1 year experience as a CNA preferred
  • State of Connecticut CNA Certification; or within 120 days of CNA Certification course completion.

Responsibilities

  • Performs basic nursing care procedures under the direction and supervision of a licensed nurse. Follow treatment plan that was designed to meet the appropriate age, needs and abilities of each resident/patient including assisting with meals and nourishment’s, transferring using proper body mechanics and appropriate assistive devices, ambulation, bathing, toileting and dressing and grooming.
  • Observes and communicates pertinent information about the adult/older adult resident/patient’s condition to the appropriate facility staff.
  • Takes and records pulse, respirations, blood pressures and weight as needed.
  • Documents care and observations on work sheets and EMR accurately and in a timely manner.
  • Maintains cooperative relationships to enhance the efficient operation of the care team by providing support and assistance to co-workers.
  • Adheres to infection control practices.
  • Supports the psychosocial needs of the residents as per the plan of care and profile.
  • Participates in educational and inservice meeting to update knowledge of techniques, guidelines and regulations. Must attend all mandatory programs. Meets basic requirement for 12 hours of continuing education per year.
  • Uses equipment in an appropriate and safe manner. Adheres to standards of the Diligent Minimal Lift Program.
  • Create and implement activity plans for residents, in the way of small groups, special events and entertainment. Motivates residents for participation in either planned groups or on a one-to-one basis.
  • Engages residents in activities to improve residents’ physical, social and emotional well-being
  • Enhances the quality of life of residents through engagement while promoting social interaction amongst residents
  • Observe and maintain safety of residents in falls program
  • Performs other duties as required.
