The HH & Help at Home CNA is responsible for providing personal care and other services outlined in the Plan of Care to patients/clients in a manner conducive to their safety and comfort. Care is provided in accordance with the policies of the agency and the nursing plan of care for the patient/client and the manager. Skillfully assist patients by providing personal services, such as, bathing, dressing, and grooming as outlined on the plan of care necessary to prevent or postpone hospitalization. Helps family members care for the patient by demonstrating appropriate ways to lift, turn, and re-position the patient. Assist families to maintain their normal and usual activities of life. Assist in re-establishing activities of daily living encouraging as much independence as possible. Observe for incidental needs of the patient/client. Exercise initiative and judgment in performance of duties. Maintain charting and records as required by the agency and federal and state regulations, assuring that charting follows the plan of care established by the case manager. See referred patients as directed by the Case Manager.
