About The Position

5 Star Cares is seeking experienced and compassionate caregivers to support higher-acuity and Level 2 residents in our group home settings.
This opportunity is ideal for professionals with hands-on experience in acute care hospitals, rehabilitation facilities, or skilled nursing environments who are passionate about delivering safe, high-quality, person-centered care.

Requirements

• Minimum 2 years experience in acute care, rehab, or skilled nursing
• Experience working with higher-acuity / Level 2 patients
• Current BLS/CPR & First Aid certification (AHA or Red Cross)
• Strong communication, observation, and documentation skills
• Ability to assist with transfers and lift 50+ lbs
• Must pass ACHA Level 2 Background Screening

Nice To Haves

• Experience with behavioral health or developmental disabilities preferred

Responsibilities

• Assist residents with personal care including bathing, grooming, feeding, and mobility
• Support activities of daily living (ADLs) with dignity and respect
• Document care accurately and communicate with the care team
• Assist individuals with intellectual disabilities and/or behavioral health needs
• Maintain a safe, clean, and supportive environment
• Safely operate standard patient care equipment
