Requirements

  • Current Certified Nursing Assistant (CNA) certification in the state of employment.
  • Minimum of 2 years’ experience as a CNA; hospice experience preferred.
  • Strong organizational and communication skills, with the ability to work independently.
  • Compassionate and patient, with the ability to support patients, families, and caregivers.
  • Ability to handle sensitive issues such as end-of-life care with professionalism and empathy.
  • Must be dependable, flexible with work hours, and able to participate in weekend rotations.
  • Proficient in documentation, scheduling, and basic computer skills.
  • Current CPR certification and a valid driver’s license with reliable, insured transportation.

Responsibilities

  • Coordinating and scheduling CNA patient visits based on physician orders and care plans.
  • Communicating with CNAs daily to confirm availability, share updates, and reassign visits as needed.
  • Maintaining and updating the master calendar of aide visits, ensuring accurate documentation.
  • Partnering with nursing supervisors to meet staffing requests with qualified personnel.
  • Providing timely communication to patients, families, and caregivers regarding scheduling changes.
  • Performing daily audits of visit frequency and documentation to ensure compliance with care plans.
  • Coordinating monthly CNA in-services and mentoring newly hired aides, including ride-alongs.
  • Stepping in to provide direct CNA care when staffing needs require support.
