About The Position

Provides hands-on care and daily support to older adults enrolled in the PACE (Program of All-Inclusive Care for the Elderly) program, helping them remain safe and independent in their community. The role includes assisting participants with activities of daily living such as bathing, dressing, grooming, toileting, mobility, meals, and hydration, as well as escorting them to activities, therapies, and appointments within the center. CNAs/CMAs monitor participants for changes in health or behavior and report concerns to the nursing team, while also offering companionship and ensuring comfort, safety, and dignity. Those certified as CMAs may additionally assist with medication administration under supervision and according to state regulations. The position is typically part of a multidisciplinary care team and is often based in a weekday, daytime PACE center environment, with some roles including limited in-home or community support.

Requirements

  • Current Certified Nursing Assistant (CNA) certification and Certified Medication Aide (CMA) certification, per state requirements
  • Active certification in good standing
  • Ability to assist with activities of daily living such as bathing, dressing, toileting, mobility, and feeding
  • Basic knowledge of elder care and patient safety practices
  • Strong communication and interpersonal skills
  • Ability to observe, document, and report changes in participant condition to nursing staff
  • Physical ability to lift, transfer, and assist participants as needed
  • Ability to work effectively as part of an interdisciplinary care team

Nice To Haves

  • Experience working with elderly adults, especially those with chronic conditions or mobility limitations
  • Previous experience in a PACE program, adult day center, long-term care, or healthcare setting
  • CMA certification if applying primarily as a CNA, or vice versa
  • Familiarity with medication administration and documentation for CMA roles
  • Experience supporting individuals with dementia or cognitive impairments
  • CPR and First Aid certification
  • Strong commitment to person-centered, compassionate care
