About The Position

Responsible for managing payer and internal gap-in-care lists to improve quality performance and patient outcomes. This role conducts patient outreach; schedules needed services and supports closure of preventive and chronic care gaps in alignment with value-based care initiatives. Provides essential support to clinical staff by assisting with routine medical procedures and patient care activities within an outpatient clinic environment. Working as part of a collaborative and well-organized care team, this role supports smooth clinic operations by preparing patients for examinations, assisting clinicians during minor procedures, and helping maintain a clean, organized, and patient-centered setting. In a noncritical care environment, the CNA Clinic contributes to efficient workflows, positive patient experiences, and high-quality care delivery through consistent communication, attentiveness, and professional conduct. Through teamwork and reliable support, this position plays an important role in ensuring coordinated and compassionate outpatient care.

Requirements

  • Current Certified Nursing Assistant registration by state of North Dakota
  • CPR certification required within 30 days of hire
  • Strong communication
  • EHR experience
  • Organizational skills

Responsibilities

  • Reviews and works assigned gap-in-care lists from payers and internal reporting systems.
  • Contacts patients via phone, mail, or electronic communication to schedule appointments and close care gaps.
  • Coordinates scheduling for services such as annual wellness visits and screenings.
  • Documents outreach attempts and outcomes
  • Collaborates with clinic staff and providers to ensure timely gap closure.
  • Identifies barriers to care and escalates appropriately.
  • Maintains knowledge of quality measures (HEDIS, CMS, payer-specific metrics).
  • Supports population health and value-based care initiatives.
  • Adheres to Trinity Health policies and confidentiality standards.
