POSITION SUMMARY: To meet the physical and psychosocial needs of the residents based on the plan of care and instruction of staff and/or nurse.
Requirements
- Current PA CNA certification is required.
- A graduate of an accredited Nurse Aide program is required.
- Current PA CNA license is required

- Previous experience in a skilled nursing/long term care facility is preferred
Responsibilities
- Report at start of shift with pocket care plans/worksheet to receive shift report.
- Prioritize care with co-workers in assigned neighborhood based on resident needs.
- Regular attendance is an essential function.
- Assist resident with the activities of daily living including but not limited to bathing, dressing, mouth care, toileting and transfers.
- Responsible for appropriate documentation of activities.
- Inform nurse of any findings or observations while caring for resident i.e. skin tears, bruises, accidents, incidents, change in status.
- Ensure competency by completing state mandated 12 hours of in-services yearly.
- Awareness of safety hazards by maintaining a clean, neat and clutter free environment.
- Maintain friendly, professional attitude with residents, visitors and staff.
- Attend Resident Care Conference for neighborhood.
- Assist with vital signs.
