Midland Care Connection Midland Care Connection is a not-for-profit, community-based organization dedicated to helping older adults and seriously ill individuals stay independent and supported in their homes. Established in 1978, we offer a full continuum of care, including hospice, palliative care, home health, grief and loss counseling, and our signature PACE (Program of All-Inclusive Care for the Elderly) program. Our mission centers on "meeting individual needs through innovative care solutions," and we serve people across multiple counties in northeast Kansas. Community Care Hospice One of Midland Care Connection’s hospice programs, offered through its Community Care Hospice partnership, provides compassionate, personalized end of life care to individuals and their families in central and south-central Kansas. Through the Community Care Hospice joint venture with local nonprofit retirement and senior living communities, the program extends this expertise across multiple counties, combining skilled clinical care with emotional and spiritual support to help patients and families navigate the final stages of life with compassion and respect.
