Requirements

• Meets the training requirements of the State.
• Completed a minimum of 75 hours of classroom and supervised practical training.
• 16 hours of classroom must be completed prior to a mandatory 16 hours of practical training.
• Minimum of six months experience including providing personal care.
• At least 18 years of age.
• Ability to read and follow written instructions and document care given.
• Understands hospice philosophy, comfortable providing care to the terminally ill.
• Satisfactory references from previous employers.
• Ability to work with little direct supervision.
• Secure with issues of death/dying.
• Provides a calm manner when in a patient's home.
• Possess and maintains current CPR certification.
• Demonstrates tact, patience, and good personal hygiene.
• Strong oral and written communication skills.
• Good organizational skills.

Responsibilities

• Help with bathing, dressing, toileting, mobility, and repositioning.
• Observe and report changes in the patient's condition.
• Assist with ambulation, transfers, ROM exercises and safe equipment use.
• Help with self-administered medications per state regulations.
• Offer companionship and comfort to patients and families.
• Change linens, assist with meals, and maintain a clean space.
• Maintain accurate and timely documentation.
• Collaborate with hospice interdisciplinary team.
