About The Position

Midland Care Connection Midland Care Connection is a not-for-profit, community-based organization in Topeka, Kansas, dedicated to helping older adults and seriously ill individuals stay independent and supported in their homes. Established in 1978, it offers a full continuum of care, including hospice, palliative care, home health, grief and loss counseling, and its signature PACE (Program of All-Inclusive Care for the Elderly) program. PACE Program The PACE program is designed to help older adults remain safely in their homes rather than entering nursing facilities. It provides complete medical care, rehabilitation, medications, transportation, social services, and supportive therapies—all coordinated by an interdisciplinary care team. Participants receive personalized care plans tailored to their unique needs, along with access to a PACE center for social activities, meals, and clinical services. By integrating medical and supportive care under one program, PACE helps seniors maintain independence, improve quality of life, and continue living in their communities with dignity. Certified Nursing Assistant A PACE Center CNA helps support older adults who are enrolled in the Program of All-Inclusive Care for the Elderly. Their work typically includes assisting participants with daily activities such as bathing, dressing, grooming, and mobility, as well as monitoring vital signs and reporting any health changes to the nursing staff. They may also help with meal assistance, accompany participants during activities, and provide overall support to ensure comfort, safety, and independence in both the center and participants’ homes. Hours Monday - Friday | 8am-5pm Pay Rate CNA - $18.75+/hour | HHA - $20.15+/hour Bonuses CNA - $1,000 | HHA - $1,500 To be paid in two increments at 6 months and 1 year of employment + CNA's receive paid training to obtain their HHA license after 60 days Shift Differentials $1.00 Weekdays | 5:00pm – 8:00am $1.50 Weekends | 8:00am – 5:00pm $2.00 Weekends | 5:00pm – 8:00pm

Requirements

Current Certified Nursing Assistant (CNA) certification in the state of Kansas
High school diploma or GED
Ability to provide hands-on assistance with activities of daily living (ADLs), including bathing, dressing, grooming, toileting, and mobility
Basic knowledge of infection control, safety practices, and patient rights
Ability to observe, document, and report changes in a client’s condition to nursing staff or supervisors
Compassionate, respectful, and patient-centered approach to care
Strong communication and interpersonal skills
Ability to work independently in clients’ homes while following care plans
Physical ability to perform job duties, including lifting, bending, and standing for extended periods
Valid driver’s license, reliable transportation, and willingness to travel to clients’ homes
Ability to pass background checks, health screenings, and required trainings

Nice To Haves

Previous experience working with older adults, individuals with chronic illness, or those with mobility or cognitive impairments
Prior home health, hospice, palliative care, or PACE program experience
Experience assisting with light housekeeping, meal preparation, and medication reminders
Familiarity with interdisciplinary care teams and community-based healthcare models
CPR/First Aid certification
Demonstrated ability to provide emotional support and companionship to clients
