Black Family Development, Inc. (BFDI) is seeking a dedicated and compassionate Full-time CMO-ISP Case Manager to join our team in Detroit, Michigan. This role is pivotal in supporting youth involved in the Juvenile Justice system and their families. The Case Manager will be responsible for maintaining a caseload of youth, conducting thorough assessments, and developing service plans tailored to meet the unique needs of each family. The position requires a strong commitment to advocacy, ensuring that all youth and families receive the necessary support and resources to thrive. The Case Manager will engage in various responsibilities, including conducting youth and family assessments, linking families to community-based treatment and rehabilitative services, and monitoring their progress. This role also involves attending court sessions and preparing reports as required. The Case Manager must maintain regular in-person contact with youth and families, adhering to probation level requirements and the specific needs of the youth. Coordination of care with assigned treatment providers is essential to ensure comprehensive support. In addition to case management duties, the Case Manager will provide individual, family, and group counseling using a home-based model. They will also assist families in crisis situations, offering support for emergencies such as food, clothing, and shelter. The Case Manager will be responsible for maintaining accurate case records, submitting program billing, and ensuring all documentation is completed in a timely manner. This position requires a flexible work schedule, as it may involve unconventional hours to meet the needs of the families served.
