About The Position

Weld County’s Department of Human Services is looking for a Program Specialist to be a resource expert of our Case Management Agency (CMA). From developing training programs for new hires to performing high-level case reviews, you will ensure our team is equipped to provide life-changing support to our residents. Performs as a subject matter expert for assigned Case Management Agency (CMA) waivers and programs. Provides training on intake and eligibility, program rules and regulations, and compliance procedures for all new and existing CMA staff. Audits CMA program, performs case review and monitoring activities for quality assurance, including accuracy, compliance, and timeliness. Primary office location will be in the Greeley office. Work may involve time away from a standard office environment, which may consist of driving to and from home visits. Work may occur in various community settings that are potentially hazardous and may require additional precautions, such as personal protective equipment and/or requesting assistance from emergency medical personnel or law enforcement. Reliable, predictable attendance within department business hours of 8 a.m. to 5 p.m. Monday through Friday. After hours work situations may occur, on occasion, to provide support to members and families who need services outside of normal business hours. This position is primarily in-person. Limited telework opportunities may be approved when operationally feasible. Ideal candidate traits: 1. Requirements

  • Bachelor's Degree or 5 years of experience in the field of Long-Term Services and Supports (LTSS), which includes Developmental Disabilities. or A combination of education and relevant experience appropriate to the requirements of the position.
  • Relevant experience is defined as: (a) Experience in one of the following areas: long-term care services and supports, gerontology, physical rehabilitation, disability services, children with special health care needs, behavioral science, special education, public health or non-profit administration, or health/medical services, including working directly with persons with physical, intellectual, or developmental disabilities, mental illness, or other vulnerable populations as appropriate to the position being filled and (b) Completed coursework and/or experience related to the type of administrative duties performed by case managers may qualify for up to two (2) years of required relevant experience.
  • 1 year experience working in Case Management for Long-Term Services and Supports and/or Medicaid waiver programs.
  • Candidate must possess the ability to learn and apply new computer skills on a regular basis.
  • Candidate must possess knowledge of computer programs such as Microsoft Office Suite.
  • Candidate must possess the ability to read and comprehend instructions, correspondence and department policies and procedures.
  • Candidate must possess the ability to effectively communicate with diverse populations in a variety of settings.
  • Candidate must possess the ability to apply math concepts and calculations in the work environment.
  • Candidate must possess the ability to apply common sense to problem solving in a work environment.
  • Candidate must possess the ability to adapt to fast paced, high volume and ever-changing work environment and be able to handle high stress situations.
  • Candidate must demonstrate the ability to show initiative, meet timelines, organize work, manage time, and manage multiple priorities.
  • Candidate must pass criminal background check prior to employment start date.
  • Candidate must pass Human Services Background Checks.
  • Candidate must pass a Colorado Adult Protective Services (CAPS) check and will continue to be monitored by CAPS throughout employment.
  • Driving is essential in this position. Candidate must have a valid Driver’s License and Liability Insurance Upon Hire
  • Candidate must pass a Motor Vehicle Record (MVR) evaluation and if hired, will be subject to continuous monthly MVR monitoring throughout employment.
  • Candidate must provide own transportation and be willing to travel as needed to meet the essential functions of the position.
  • Pre-employment substance screening is required for this position.
  • This position is subject to a pre-employment drug screening.
  • Employment is contingent upon successfully passing the screening in accordance with Weld County policy.

Responsibilities

  • Stays current on all applicable rules and regulations that may impact the CMA and represents the agency at required local and state meetings, committees, workshops, and conferences.
  • Performs detailed case audits, evaluating documentation and system data to ensure accurate, timely, and regulation compliant case management.
  • Documents case review findings clearly and thoroughly for supervisory and case manager review and follow-up.
  • Shadows case management staff to observe Level of Care Screens and other required duties, ensuring adherence to state requirements and documents accordingly.
  • Delivers timely and accurate feedback to staff on case review outcomes, opportunities for improvement, and best practices.
  • Serves as a technical resource in assigned service areas, providing clarification, guidance, and support on program policies, procedures and compliance expectations.
  • Develops, implements, and facilitates comprehensive onboarding training program for new employees.
  • Creates, delivers, and documents on-going training for existing staff based on identified needs, regulatory updates, and performance trends.
  • Maintains and updates all training materials, tools, manuals, and presentations as CMA rules, guidelines, and processes change.
  • Documents training outcomes and provides supervisors with feedback on strengths, gaps, and emerging trends requiring additional training.
  • Provides training and education to other divisions and community partners as needed or requested.
  • Models high quality, person-centered service delivery, demonstrating expected interactions, practices and standards.
  • Supports staff through troubleshooting and provides guidance on resolving complex cases and navigating program requirements.
  • Collaborates with CMA leadership to develop, refine and implement internal policies and procedures that support program effectiveness and compliance.
  • Participates in meetings, trainings, and conferences related to CMA programs and the populations served to stay informed and aligned with best practices.
  • Provides direct support to case managers during periods of high intake volume or when there are temporarily uncovered caseloads.
  • Coordinates or facilitates annual Service Plan Meetings, Continued Stay Review/Level of Care Screening and Assessments, quarterly monitoring visits, program transitions, and Member Identified Team meetings, as needed to support case managers.
  • Provides assistance to other Program Specialists as workload or program needs arise.
  • May manage a caseload of complex, confidential and/or sensitive cases as assigned.
  • Maintains the provider resource lists utilized by case managers and provided to members.
  • Participates in a positive and value-added manner with both internal and external professional partners.
  • Stays informed by participating as appropriate in all team, unit, and division meetings, attends or views recordings for department updates and reads county wide messaging.
  • Participates as a team member of the department establishing and maintaining a high degree of cooperation with coworkers.
  • Displays a willingness to collaborate and problem solve.
  • Understands how one’s day to day work significantly impacts the team’s success.
