Club Coordinator (Club Director-in-Training) About Oakwood Creative Care Oakwood Creative Care (OCC) provides innovative, person-centered adult day programs for older adults and individuals living with dementia and other cognitive or physical challenges. Position Summary The Club Coordinator is a Club Director in Training Role and is designed to develop future operations leaders within Oakwood Creative Care. Under the guidance of the Club Director and Senior Club Director, this position supports day-to-day club operations, member services coordination, regulatory compliance, and staff support while cultivating an elevated, joy-centered member experience. This role balances hands-on engagement with members and increasing operational responsibility, preparing the individual to step into a Club Director role as competencies are demonstrated.

Requirements

  • Bachelor’s degree in social work, recreation therapy, gerontology, human services, or related field preferred
  • 2+ years of experience working with older adults or individuals living with dementia
  • Ability to provide direct care support as needed
  • Ability to lift up to 25 lbs

  • Experience in adult day, healthcare, or community-based services preferred
  • Demonstrated leadership potential and desire to advance into a Club Director role

Responsibilities

  • Support implementation of innovative, enriching programs aligned with OCC’s philosophy of engagement
  • Facilitate daily activities and provide direct care support as needed
  • Help ensure the monthly program calendar reflects creativity, dignity, and joy
  • Elevate the member experience by cultivating a warm, welcoming, and aesthetically uplifting environment
  • Monitor member well-being and communicate observations to the Club Director
  • Support coordination of care plans, assessments, and progress notes under Director supervision
  • Assist in overseeing day-to-day club flow and staffing coordination
  • Help monitor staffing patterns, certifications, and training compliance
  • Support membership processes including inquiries, tours, enrollments, and documentation tracking
  • Ensure member files remain current (H&P, TB tests, emergency contacts, assessments, etc.)
  • Maintain accurate attendance records and service documentation
  • Support preparation for inspections, audits, and regulatory reviews
  • Ensure compliance with: Arizona Department of Health Services (DHS) regulations Area Agency on Aging (AAA) requirements Veterans Administration (VA) contractual guidelines (as applicable) ALTCS and other payer requirements OCC policies and quality management standards
  • Help ensure the club environment meets environmental standards for safety, cleanliness, infection control, and accessibility
  • Assist with quarterly fire drills, disaster preparedness reviews, and member council meetings
  • Support incident reporting and grievance documentation processes
  • Help track outcome measurements and benchmarks using OCC-approved tools
  • Engage in quality improvement initiatives to ensure excellence in service delivery
  • Serve as a daily point of contact for members and families
  • Support relationship-building meetings with caregivers
  • Maintain awareness of community resources and program trends
  • Support OCC fundraising efforts and special events as requested
