About The Position
The Population Health Coordinator reports to the Population Health Supervisor and will perform and provide a variety of services related to the administrative function of the health system Population Health program. The Coordinator will work closely with healthcare providers and community organizations to improve health outcomes for the population served. This role involves data collection, analysis, and reporting to support the program's initiatives and objectives. The Coordinator will also assist in the development and implementation of health programs and interventions aimed at enhancing community health and wellness.
