The Clinical Care Coordinator for our brand new Community Wellness and Response Team (CWRT) is responsible for developing and providing high-quality therapeutic interventions and services in support of an individual’s wellness and recovery. Services include strength-based and needs-based therapeutic case management, problem-solving, therapeutic rehabilitation including skills-building, benefits support, resource development, housing navigation, crisis intervention, and family support. Schedules may include after hours as needed by the program. Our CWRT operate 24/7, 365; providing in-field response to de-escalate and resource community members. Responders go out as a team of 2. CWRT is formerly known as Alternatives to 911. This innovative program works collaboratively with 988 and Sacramento County to provide urgent response for community members experiencing a mental health crisis to reduce the need for emergency services and provide community based resources.
