Health Solutions is a premier wellness center focused on whole person care. With over 400 employees in Southern Colorado, you would be joining a mighty team of support and administrative staff, clinicians, physicians, nurses, and others in our efforts to improve the health and wellbeing of our community. We’re hiring Outpatient Clinicians to work with adult clients at our Crestone location in Pueblo, CO. What You’ll Be Doing RESPONSIBILITIES: As an Outpatient Clinicians under the supervision of the Director and Supervisor you will: Conducts screenings and assessments including those associated with substance abuse/mental health/co-occurring needs Provides individual, couples/family, and/or group therapies Provides case management services Maintains accurate, complete, and up-to-date written and electronic records Facilitates collaborative treatment planning with clients Completes 90% of documentation within 24-hours of service delivery Meets minimum monthly productivity standards Maintains timely caseload management by progressing clients appropriately through treatment and discharging those who have completed treatment or are no longer engaged in a suitable manner Participates in team meetings, case consultations, and other meetings as directed Works collaboratively and represents Health Solutions with community agencies and partners; and Performs other job-related duties as requested and assigned based on program and client needs. Physical requirements include the following: May stay stationary for prolonged periods of time Moves within the facility Frequently exposed to stressful or emergent situations Frequently operates a computer and other office equipment to record health data Constantly communicates with clients and others Occasionally moves equipment and/or materials up to 20 pounds Occasionally drives between Health Solutions facilities What You’ll Like About Us Competitive pay $ $60,000- $76,458 Annually (Dependent on experience, certification and shift) Generous benefits package. Requirements

  • Master’s degree in social work, counseling, or a closely related field
  • Must be willing/able to obtain candidate status with DORA within 2 weeks of hire date.
  • Experience working successfully within a multi-disciplinary team
  • Computer proficiency
  • Valid Colorado driver’s license, good driving record, and access to transportation during work hours
  • Must already be authorized to work in the US; sponsorships not available.
