Patient Care Coordination: Clinical Coordinator LPNs are responsible for coordinating patient care activities, which include scheduling appointments, managing patient records, and ensuring that healthcare practices comply with established standards. Patient care as needed.Communication Liaison: They act as a bridge between patients, families, and healthcare providers, facilitating communication to ensure comprehensive care delivery. This includes addressing patient inquiries and concerns.
