About The Position

Under administrative guidance, the Clinical Pharmacist Transplant will provide comprehensive pharmacy care services relating to transplant, including the review of appropriate drug therapy, consultations with physicians for each transplant recipient, and reviewing dose and inter-patient variables in all transplant patients. This pharmacist will provide thorough medication education to all transplant recipients prior to discharge from the hospital and will actively participate in daily rounds with the Transplant team. The role is crucial in the coordination and delivery of patient care in collaboration with transplant surgeons, nephrologists, and a multidisciplinary team including physicians, nurses, clinical coordinators, social workers, financial coordinators, and administrative personnel. The pharmacist will engage in all phases of the transplant process, from selection and discharge planning to post-transplant care, ensuring a seamless transition and continuous care for both recipients and donors. Attendance and participation in selection committee meetings and inpatient rounding with the multidisciplinary team are essential. The pharmacist will also actively participate in outstanding customer service and accept responsibility in maintaining relationships that are equally respectful to all, ensuring an integrated approach to care delivery for transplant patients.

Requirements

  • PharmD from college of pharmacy.
  • Licensed Pharmacist (RPH).
  • Experience in organ transplantation.
  • 1 year of experience as a clinical pharmacist or completion of a Transplant residency PGY1.
