About The Position

The Clinical Transplant - Coordinator is responsible for coordinating a multidisciplinary approach in the evaluation, treatment, and facilitation of the transplant process. This role encompasses providing follow-up care for organ recipients and donors, serving as both a collegial and consultative resource for nursing leadership, primary nurses, and other healthcare team members. The Coordinator promotes seamless communication, individualized care planning, and continuous quality improvement within the transplant program.

Requirements

  • Demonstrated clinical competency and leadership skills through relevant experience.
  • Prior clinical experience in transplant programs or related specialties is required.
  • Strong organizational, interpersonal, verbal, and written communication skills.
  • Critical thinking and problem-solving abilities to manage complex patient scenarios.
  • Ability to work independently and efficiently manage multiple tasks.
  • Project management skills to oversee various aspects of clinical coordination.
  • Associate of Nursing (ASN) required; Bachelor of Nursing (BSN) preferred.
  • New ASN RNs hired after January 1, 2013, must complete their BSN within five (5) years of hire.
  • Active RN license in Indiana or Nurse Licensure Compact (NLC).
  • Basic Life Support (BLS) certification through the American Heart Association (AHA).
  • Additional advanced life support certifications may be required based on unit/department policies.

Responsibilities

  • Oversee the comprehensive management of transplant patients from evaluation through follow-up care.
  • Collaborate with a multidisciplinary team to ensure effective communication and seamless care delivery.
  • Facilitate communication among nursing leadership, primary care nurses, and other healthcare professionals to optimize patient outcomes.
  • Develop and implement individualized care plans for organ recipients and donors based on specific needs and preferences.
  • Track patient progress, coordinate appointments, and ensure timely delivery of services using strong organizational skills.
  • Advocate for patients to ensure they receive holistic care and support throughout their transplant journey.
  • Contribute to quality improvement initiatives aimed at enhancing transplant services and clinical practices.
