About The Position

The Clinical Transformation Manager (CTM) at UnitedHealthcare is responsible for the ongoing clinical management of physician practices involved in various healthcare programs. This role focuses on leading practice transformation and performance improvement initiatives, ensuring practices achieve targeted goals related to quality, efficiency, and utilization. The CTM serves as the primary clinical contact for practices, communicating progress to senior leadership and driving transformational change within the healthcare system.

Requirements

  • 4+ years of experience in improving clinical quality, healthcare analytics, or driving clinical transformation initiatives.
  • Experience in interpreting and utilizing clinical data analytics and outcomes measurement in healthcare.
  • Experience collaborating with senior level leadership.
  • Demonstrated experience leading groups and solid presentation skills.
  • Solid Microsoft Office skills (Word, Excel, Outlook, PowerPoint).
  • Strong relationship-building skills with internal and practice teams.
  • Access to high-speed internet for home office setup.
  • Willingness to travel up to 10% for face-to-face meetings.
  • Access to reliable transportation for travel to client and/or patient sites.

Nice To Haves

  • Experience implementing Clinical Practice Transformation initiatives for provider or hospital groups.
  • Experience preparing and presenting information to clinical and executive level leadership.
  • Physician practice experience.

Responsibilities

  • Accountable for successful deployment of UHC's Clinical Support program at the practice level.
  • Build and maintain relationships with practice leadership and key clinical influencers.
  • Facilitate practice improvement meetings to monitor and discuss progress on transformation action plans.
  • Develop strategies for improvement based on performance analysis, including specific outcomes and metrics.
  • Design and implement performance improvement initiatives to assist practices in achieving transformation milestones.
  • Monitor and review practice progress in milestone achievement and ensure accountability.
  • Analyze key cost, utilization, and quality data to assess practice performance.
  • Work with stakeholders to implement proactive strategies to address issues and measure impact using PDSA methods.
  • Assist practices in creating workflows to optimize care delivery and evaluate outcomes.
  • Educate and deploy technology tools to support practice transformation.
  • Integrate technology tools into practice workflows.
  • Collaborate with various UnitedHealthcare teams to support integrated care for members.
  • Consult with practices to identify challenges hindering program outcomes and develop innovative solutions.
  • Align and integrate Optum care/disease management programs with practice capabilities.
