Concordia Visiting Nurses is a Medicare-certified, state licensed, Medicaid approved home health agency operating in nine Western Pennsylvania counties. For well over a decade, Concordia Visiting Nurses has provided high-quality skilled nursing, palliative care, and a variety of therapy services to our patients. Requirements

Current PT or OT Allied Health licensure (as applicable)
3+ years of clinical experience, with at least 1 year in a leadership or supervisory role
Strong interpersonal and communication skills
Passion for team development and delivering exceptional care
Organized, proactive, and calm under pressure

Responsibilities

Leading and supporting a dedicated clinical team
Overseeing day-to-day clinical operations and ensuring compliance with care standards
Mentoring, training, and empowering staff
Collaborating with leadership to improve patient outcomes and satisfaction
Fostering a respectful, positive work culture where everyone feels heard
