Job Summary Provides psychosocial evaluations, psychotherapeutic interventions, case management and discharge planning services for patients and/or their families. Provides and acts as a liaison for physician, patient and/or familiy between the hospital, community agencies and facilities. Accountabilities Performs intake assessment/evaluation of family dynamics and treatment needs, including creation of treatment plan. Prepares case history of patient's personal, social, and emotional development. Assists patients to evaluate both their strengths and symptoms, and facilitate patients setting their own goals, and plan for appropriate services. Provides therapeutic intervention with patient family including individual, marital, parental, family and group therapies. Interprets nature of illness and prescribed course of treatment to patient and families. Communicates family issues to other staff members. Helps patients develop social skills, interests, and leisure time activities to provide a sense of participation and personal worth, including opportunities for age appropriate, culturally appropriate activities. Assists patients to identify appropriate employment opportunities and vocational rehabilitation services. Functions as Designated Examiner which involves the commitment process, designated exam, provide testimony for probate courts. Supervisory/Management Responsibilities Leads the treatment team in interdisciplinary planning. Provides leadership with the flow of information among team members and payer source. Leads support groups, patient/family education. Develops and achieves annual goals. Facilitates family involvement in the treatment process. Works with referral sources, agencies and departments within the GHS system to promote clinical services at local and regional conferences. Promotes public awareness of clinical services through educational conferences, local events, and educational institutions. Promotes the organization by ensuing an understanding of program services available for patients; publicizing activities and accomplishments; adhering to a professional code of ethics. Assists student interns from affiliated schools in development of professional and clinical skills. Determines presence of appropriate discharge placement and feasibility of caregivers participation in the discharge process. Works with Utilization Review and case managers/ third party payer and hospital business office to negotiate coverage for psychiatric treatment when services are not a part of the clients covered services. Assess patient's recommended discharge follow up services (medication, physician services, therapies, environmental needs /housing accessibility) with available resources. Enhances professional growth and development through professional affiliations; participation in educational programs, in service meetings, workshops and review of current literature. Participates in planning conferences and peer supervision. Provides departmental support and other responsibilities as warranted.

Requirements

  • Master's Degree - Social Work or Counseling, Psychology or Related Degree
  • 1 year of Behavioral health/counseling psychology experience
  • Medical Social Worker MSW OR Licensed Social Worker CP LISW-CP OR Licensed Social Worker LISW-AP OR Licensed Professional Counselor LPC or LPCI (Lawson Code: COTR)

  • Basic Computer Skills

Nice To Haves

  • Inpatient psychiatric hospital experience
  • clinical assessments and case management experience preferred.
