About The Position

HealthRIGHT 360, a nonprofit organization and a family of programs, is committed to providing accessible and comprehensive healthcare services to vulnerable populations. Our mission is to tackle systemic barriers to healthcare and promote health equity for all. We offer a wide range of services, including mental health care, residential and outpatient substance use treatment, and primary health services.Additionally, we provide transitional support for individuals re-entering the community after involvement in the criminal justice system. By integrating physical and behavioral health, we empower individuals to overcome challenges by addressing social determinants of health, fostering resilience, and facilitating recovery. The Clinical Therapist is a BBS Registered/Licensed individual who provides mental health services including substance use individual and group therapy based on individualized assessments in a school and outpatient setting. The intensity of services is based on the client's stage of change and response to treatment. The Clinical Therapist collaborates closely with school staff, families, and multidisciplinary teams to support students’ mental health within the educational environment. Additional responsibilities may include providing clinical oversight for non-licensed staff, supporting interns, contributing to trainings, and participating in regular supervision and licensure-specific professional development. Depending on program needs, the role may also involve participating in an after-hours crisis response rotation.

Requirements

  • Master’s degree in counseling, social work, or related field.
  • Current Behavioral Board of Sciences (BBS) registration/license from an accredited institution recognized in California(LMFT, LPCC, LCSW).
  • First Aid and CPR Certification.
  • Valid California driver’s license.
  • Dependable means of transportation to various school sites.
  • Group facilitation experience.
  • Experience with youth populations.
  • Experience with Crisis Intervention.
  • History of meeting or exceeding Fee for Service expectations.
  • Experience in coordination within school systems
  • Knowledge of mental health and trauma informed treatment.
  • Knowledge of cultural stigmas of counseling and treatment services.
  • Knowledge of principles and techniques of the healing arts, case management, crisis intervention, client centered counseling, and group facilitation.
  • Strong proficiency with Microsoft Office applications, specifically Word Outlook and internet applications.
  • Knowledge and experience with Medi-Cal documentation standards preferred.
  • Knowledge of substance abuse recovery principles and resources; including Co-occurring disorders preferred.

Responsibilities

  • Conducts intakes, comprehensive assessments, individualized treatment plans, and provides individual counseling for students and clients in both school and outpatient settings.
  • Delivers developmentally appropriate therapeutic interventions to support emotional, behavioral, and academic or personal growth.
  • Connects students/clients with relevant internal services and external community resources, coordinating care and following up on progress to ensure continuity of treatment.
  • Facilitates group counseling and psychoeducational sessions tailored to the needs of children, adolescents, and families in school and outpatient environments.
  • Participates in or leads school-based and community outreach initiatives, including mental health awareness and wellness education workshops.
  • Responds to crises by conducting risk assessments, providing immediate stabilization support, and collaborating with school staff or outpatient team members to ensure safety and access to necessary services.
  • Maintains compliance with agency, educational, and legal documentation requirements by attending trainings, team meetings, and supervision.
  • Assists in planning and coordinating student/client-centered events such as wellness days, graduation celebrations, and family engagement activities.
  • May provide after-hours or weekend support on a rotating basis for crisis stabilization, school or community events, and outreach efforts as needed.
  • Collaborate with each caseload client and other available internal and external resources to develop/maintain treatment plans, transition plans, progress notes and appropriate updates insupport of the health and recovery needs of the client.
  • Properly document all individual and group counseling sessions, attempts to contact, and complete the discharge paperwork/process and required agency assessments in timely manner.
  • Maintain accurate records by data entering documentation into various electronic systems for all caseload clients in accordance with guidelines established by HealthRIGHT 360 to satisfy internal and external evaluating requirements.
  • Participate in Peer Chart/ Utilization Reviews for preparation of program audits.
  • Will adhere to documentation policies and procedures set forth by Compliance Department.
  • Be responsive and pro-active in all correspondences with referring parties such as behavioral health providers, probation, social workers, and the Quality Improvement Dept. via treatment status reports, phone calls, faxes, etc.
  • Provide informed, written status reports to the above entities as appropriate.
  • Assist in providing written progress reports/narratives to Clinical Program Manager or Directors as requested.
  • Must achieve 50% of service delivery goals while delivering high-quality care and services.
  • May be required to meet service delivery goals at other Santa Clara County Behavioral Health programs.
  • Ensures coverage during absences or unplanned events by coordinating with team members and management.
  • Adjusts workload as necessary to ensure uninterrupted service delivery, including stepping in to provide support during peak demand times or staff shortages.
  • Adheres to the program’s scheduling protocols to maintain appropriate client-to-provider ratios.
