About Us: Growing Hope is dedicated to providing exceptional mental health and behavioral health services to at risk children, youth and their families. We are seeking a passionate and experienced therapist to join our team and make a meaningful impact in the community. Key Responsibilities: Provide individual and family therapy to children and families. Conduct assessments, Individual Plans of Care, and manage crisis situations. Collaborate with inter and intra agency staff and other professionals to ensure comprehensive care. Utilize a secure telehealth platform for remote sessions, with occasional in-person sessions as needed. Maintain detailed documentation in compliance with Growing Hope and Medicaid regulatory standards. Essential Duties: Manage a panel of clients, delivering evidence-based assessments and therapeutic interventions. Develop and update clinical and Independent Plans of Care in collaboration with clients, family members and other assigned providers. Track client progress and serve as an accountability partner. Engage in discharge planning, case management, and coordination with other providers. Respond to crises, develop safety plans, and provide follow-up intervention services. Communicate effectively with team members and report any clinical concerns to the Clinical Director. Lead treatment team meetings and apply an integrated care model to enhance treatment outcomes. Submit documentation of all services in accordance with regulations determined by Growing Hope, South Carolina Department of Health and Human Services, and other regulatory authorities Adhere to ethical standards and agency procedures for abuse reporting and crisis situations. Other duties as assigned.
