This position provides community based clinical services through the FACT program to support individuals with severe and persistent mental illness in achieving stability and independence. Deliver therapy, conduct assessments under supervision, and collaborate with multidisciplinary team to coordinate comprehensive care.

Requirements

  • Bachelor’s degree in social work (master’s preferred) or master’s degree in Counseling.
  • Current Ohio licensure: LSW or LPC (LISW/LPCC preferred)
  • Minimum 1 year of experience working with SPMI clients
  • Valid driver’s license and ability to work flexible hours
  • Must demonstrate knowledge of mental illness, individual, group counseling skills, assessment skills, and crisis intervention skills.
  • Must demonstrate the ability to define problems, analyze data, establish facts and draw valid conclusions and make recommendations.
  • Must demonstrate a positive attitude, commitment to provide superior service, and enthusiastic support for the mission of the agency.
  • Must demonstrate personal integrity, the ability to develop and work as part of a team, and a commitment to the mission of the agency.
  • Must be able to work with ill, disabled, emotionally upset and at times hostile individuals.
  • Must be able to drive and be insured under the agency’s commercial liability policy as a driver if hired in a program where driving is required.

Responsibilities

  • Provides diagnostic service via face-to-face contact with clients under the supervision of a licensed independent clinician or via telehealth under the supervision of a licensed independent clinician.
  • Provides treatment planning and counseling services to an assigned caseload of clients in accordance with state mandates, established agency and program policies, procedures, and philosophy.
  • Documents all services provided and maintains records utilizing the electronic record in accordance with agency policy/procedure.
  • Office-based case management as needed.
