About The Position

This position provides direct service to patients in a Behavioral Health Program area. The role is full-time with exact scheduling based on clinic needs.

Requirements

  • Master's Degree in Counseling, Social Work, Psychology or other related field.
  • APSW, LPC-IT, or LMFT-IT certification.
  • TACT-Level 2 certification.
  • CPR/BLS preferred.
  • Passing the Driver's License Check and/or Credit Check (for those positions requiring).
  • Passing the WI Caregiver Background Check and/or IL Health Care Workers Background Check.
  • Must be able to follow written and oral instructions.

Responsibilities

  • Completes psychosocial assessments and treatment plans in cooperation with patient and multidisciplinary team.
  • Conducts a comprehensive assessment of the clinical needs of each patient prior to beginning individual, family or group therapy.
  • Works collaboratively with the patient to develop a treatment plan addressing patient's identified treatment goals.
  • Delivers behavioral health care in accordance with patient evaluation, treatment plan and within the scope of their license.
  • Works directly in treatment relationship with patients, individually or in groups, in consultation with Psychiatrist, NP.
  • Provides crisis assessment and intervention.
  • Consults and coordinates care of patient with physician, NP, therapists and community agencies.
  • Provides appropriate discharge planning.
  • Refers patient or patient's family to other community resources.
  • Participates in clinical collaboration and or psychiatry supervision to discuss cases and determine appropriate course of treatment.
  • Maintains pertinent patient data and documentation in medical record in accordance with state, TJC and Mercyhealth system guidelines.
  • Attends required meetings or staffing as indicated by Program Manager.
  • Participates in performance improvement activities.
  • Performs other duties as assigned.
  • Complies with all Mercy Health System, State and JCAHO policies and regulations.
