About The Position

ESSENTIAL DUTIES AND RESPONSIBILITIES Completes psychosocial assessments and treatment plans in cooperation with patient and multidisciplinary team. Conducts a comprehensive assessment of the clinical needs of each patient prior to beginning individual, family or group therapy. Works collaboratively with the patient to develop a treatment plan addressing patient’s identified treatment goals. Delivers behavioral health care in accordance with patient evaluation, treatment plan and within the scope of their license. Works directly in treatment relationship with patients, individually or in groups, in consultation with Psychiatrist/NP. Provides crisis assessment and intervention. Consults and coordinates care of patient with physician/NP, therapists and community agencies. Provides appropriate discharge planning. Refers patient or patient's family to other community resources. Participates in clinical collaboration and/or psychiatry supervision to discuss cases and determine appropriate course of treatment. Maintains pertinent patient data and documentation in medical record in accordance with state, TJC and Mercyhealth system guidelines. Attends required meetings or staffing as indicated by Program Manager. Participates in performance improvement activities. Performs other duties as assigned. Complies with all Mercy Health System, State and JCAHO policies and regulations. Requirements

  • Master's Degree in Counseling, Social Work, Psychology or other related field and appropriate level of field experience and coursework.
  • Licensed to independently provide psychotherapy in Wisconsin (LCSW, LPC, LMFT)
  • TACT Certified Level II (as assigned by supervisor)
  • CPR Certified (as assigned by supervisor)
  • Passing the Driver’s License Check and/or Credit Check (for those positions requiring).
  • Passing the WI Caregiver Background Check and/or IL Health Care Workers Background Check.
  • Must be able to follow written/oral instructions.
