About The Position

The Clinical Team Manager Registered Nurse (RN) position at WellMed, part of the Optum family, involves overseeing and coordinating clinical disciplines within the home health department. This role is crucial for ensuring the delivery of quality patient care, managing staff, and maintaining compliance with healthcare regulations. The Clinical Team Manager will work collaboratively with healthcare professionals to enhance patient outcomes and will be responsible for staff recruitment, training, and performance monitoring.

Requirements

  • Graduate of an accredited school of nursing.
  • Current licensure as a Registered Nurse in Texas.
  • Ability to maintain Basic Cardiac Life Support certification.
  • 3+ years of experience as an RN in an acute care or general medical/surgical setting.
  • Familiarity with medical terminology and equipment.
  • Proven organization, communication, and human relations skills.
  • Self-motivation and independent decision-making skills.
  • Intermediate PC skills (Excel, PowerPoint, Word, etc.).
  • Ability to travel as required.

Nice To Haves

  • Experience in a community health or home health setting.
  • Supervisory experience.
  • Knowledge of general office procedures.
  • Excellent computer skills and knowledge of word processing and Excel.
  • Home health experience.

Responsibilities

  • Manage the recruitment, hiring, and development of clinical and non-clinical staff to ensure quality care delivery.
  • Participate in the orientation and training of clinical and paraprofessional staff.
  • Ensure timely on-site supervisory requirements for designated staff are met.
  • Demonstrate effective leadership and problem-solving skills.
  • Assure clinical staff perform accurate assessments and communicate effectively with physicians.
  • Manage staff assignments and monitor daily and weekly schedules.
  • Ensure patient/client clinical records meet legal and regulatory requirements.
  • Develop knowledge of services and resources available within the company and community.
  • Ensure execution of clients' plans of care as per physician orders.
  • Conduct annual evaluations of staff according to policies and regulations.
  • Supervise clinical processes and ensure proper documentation flow from inquiry to discharge.
  • Provide support for reimbursement documentation and facilitate data collection for improvement.
  • Coordinate patient services and participate in interdisciplinary care conferences.
  • Monitor documentation to ensure compliance with Medicare and payer guidelines.
  • Investigate patient/family complaints and take corrective action as needed.
  • Make home visits to provide patient care as necessary.
  • Participate in accreditation and licensing endeavors according to requirements.
