Concordia Visiting Nurses is a Medicare-certified, state licensed, Medicaid approved home health agency operating in nine Western Pennsylvania counties. The assistant works closely with the clinical team by assisting with various non-clinical duties by proving administrative support for daily operations by coordinating, monitoring and evaluating options and services for patients and clinical staff. Requirements

Two (2) years of progressive experience in a home health care setting preferred
Interpersonal, communication and organizational skills
Proficient and knowledgeable of an electronic medical record
Knowledge base of agency policies, federal and state regulations and third party payers
Reliable, independent means of transportation. Drivers must have a valid driver’s license and drive a vehicle with insurance limits as established by the state

Responsibilities

Reviews clinical documentation for compliance and completeness in a timely manner.
Reviews OASIS for accuracy and completeness.
Notifies the primary clinician of the need for recertification/discharge decision in a timely manner.
Knowledge about payer processes and requirements to bridge the gap between provider and payer and to effectively meet the needs of both, ultimately for the patient’s benefit.
Facilitates communication between patient/family, physician and all other healthcare providers.
Assimilate data for physician practices and others as requested.
