Concordia Visiting Nurses is a Medicare-certified, state licensed, Medicaid approved home health agency operating in nine Western Pennsylvania counties. We strive to hire excellent people at Concordia - and keep them as well! Concordia Visiting Nurses is seeking a Clinical Manager Assistant for our Cabot office location. The assistant works closely with the clinical team by assisting with various non-clinical duties by proving administrative support for daily operations by coordinating, monitoring and evaluating options and services for patients and clinical staff. Requirements

  • Two (2) years of progressive experience in a home health care setting preferred
  • Interpersonal, communication and organizational skills
  • Proficient and knowledgeable of an electronic medical record
  • Knowledge base of agency policies, federal and state regulations and third party payers
  • Reliable, independent means of transportation. Drivers must have a valid driver’s license and drive a vehicle with insurance limits as established by the state

Responsibilities

  • Reviews clinical documentation for compliance and completeness in a timely manner.
  • Reviews OASIS for accuracy and completeness.
  • Notifies the primary clinician of the need for recertification/discharge decision in a timely manner.
  • Knowledge about payer processes and requirements to bridge the gap between provider and payer and to effectively meet the needs of both, ultimately for the patient’s benefit.
  • Facilitates communication between patient/family, physician and all other healthcare providers.
  • Assimilate data for physician practices and others as requested.
