About The Position

Position Purpose: As a Clinical Support Specialist, you’ll play a vital role in supporting Nursing Services by ensuring accurate documentation, smooth resident transitions, and efficient day-to-day neighborhood operations. This position is key to keeping our clinical teams organized and our residents well-supported — all while living out the Mission, Vision, and Values of Trinity Health Senior Communities What you will do: Support nursing staff with accurate medical and administrative documentation. Maintain daily census reports and current resident rosters. Coordinate resident admissions, transfers, and discharges. Schedule appointments and assist with transportation arrangements for diagnostic and therapeutic services. Ensure medical records are complete, updated, and properly routed to Medical Records. Enter documentation accurately into Care Tracker and other electronic systems. Communicate diet changes and important updates to appropriate departments. Answer phones, route calls, manage faxes, and provide general administrative support to the neighborhood. Serve as a key communication link between nursing staff, residents, families, and other departments. Participate in ongoing education and required trainings.

Requirements

  • High school diploma or equivalent required.
  • Knowledge of medical terminology preferred.
  • Experience in long-term care or working with older adults preferred.
  • Proficiency with Microsoft Office (Word, Excel, PowerPoint) and electronic systems.
  • Strong organizational skills with attention to detail.
  • Excellent verbal and written communication skills.
  • Professional, customer-service focused approach when working with residents, families, and staff.
  • Ability to multitask and thrive in a fast-paced, team-oriented environment.
  • Self-motivated with strong accountability and sound decision-making skills.

Nice To Haves

  • Knowledge of medical terminology preferred.
