At Healthcare in Action, we serve patients experiencing homelessness through an innovative “street medicine” approach by contracting with Medicare and Medicaid health plans as well as hospitals and health systems. Street medicine entails using a medically equipped van to deliver medical care directly to people experiencing homelessness in locations such as parking lots, encampments, and parks. The Clinical Support Partner (MA or EMT) will support clinicians in monitoring patients who are experiencing or are at risk of experiencing homelessness with managing their own health by performing basic clinical care such as obtaining vitals, drawing blood and performing basic wound care. You will coordinate care for patients, including requesting ride-share services for patients, picking up prescriptions and scheduling appointments. Assist in inventory of the stock room, ordering supplies and fielding and triaging patient phone calls and new consults.
