LPN- Clinical Support In office role 4 days per week Shift Option: 7:00am - 4:00pm, 8am - 5pm or 9am - 6pm No holidays/weekends FUNCTIONS: Draws blood specimens for lab testing as per physician order, in collaboration with Team Leaders and the Clinical Support Team. Completes requisite specimen forms and delivers blood specimens to community labs for processing. Follows up with attending physician’s office to confirm receipt of results and communicates the results to the appropriate team. Documents in an accurate and concise manner on the day the service was provided; this includes but is not limited to lab values, telephone calls, modified orders and team notification. Monitors the scheduling of ongoing lab draws Supports functions by answering calls from patients/families/staff immediately. Follows up after hour calls/visits/client issues and concerns as requested. Provides visit for new symptom, death or crisis or routine needs as requested. Informs other disciplines and volunteers on team of changes in condition, goals and plan of care. Demonstrates thorough knowledge of current palliative care protocols for symptom management. Maintains electronic patient records. Communicates effectively with organizational staff, physicians, patients, families, nursing home staff, outside contractors, community agencies. Provides education to patients and caregivers regarding diagnosis, care, treatments, medications, and end of life expectations in a method or manner that they understand to equip them with the information they need to feel confident in the plan of care. Honors and respects the customs, choices, needs and goals of each patient and family. Understands reimbursement and payer source implications to the plan of care and utilizes resources wisely and effectively. Maintains a safe environment by utilizing and adhering to safe work policies and practices. Attends all mandatory meetings and educational programs and participates in Quality Assurance and Performance Improvement (QAPI) activities as assigned. Assists with the orientation of new staff and visiting professionals. Assists in program planning for organizational, community groups, and visiting professionals.

Requirements

  • LPN licensed in Pennsylvania
  • Drivers license and your own vehicle required
  • Minimum of two years hospice, med/surg, home health or relevant exp. required
  • Must be able to complete 5- 6 weeks of a FT (40 hr./wk.) orientation.
  • Must work a minimum of 64 hours per two-week pay period
  • Must work a minimum of 40 hours per two week pay period

Nice To Haves

  • Phlebotomy experience preferred
