We seek a Dynamic, self-motivated Clinical Support Coordinator who thrives in a fast-paced environment and supports our mission . Responsible for the coordination and delivery of clinical care to patients and families in the HSJ service areas in accordance with Federal and State standards and laws. Schedules the assignments of the assigned staff. Under the direction of the CCO, DHCS, the Clinical Support Coordinator will handle phone calls from assigned staff. Assist with care coordination as requested to back up the Team Lead and triage needs appropriately. The Clinical Support Coordinator is responsible for ensuring the compliance of patient visit schedules, quality assurance activities, and coordination of care in the absence of the assigned staff. Coordination of Services: Responsible for staff work schedules, including approval of PTO Overtime, and adjusted work schedules to meet patient care needs. Monitors time and attendance of assigned staff and addresses areas of concern as appropriate. Assist with employee performance improvement activities for assigned staff. Collaborates with the DHCS and TL to ensure tracer visits are completed on time and in accordance with agency standards. Provide input to TL of assigned staff, Annual Performance Evaluations. Responsible for the productivity of assigned staff while maintaining productivity levels that are within the NHPCO standards, while meeting patient needs and maintaining patient safety. Assists with Medline supply approvals. Monitor daily visits of field staff. Triage calls from patients, families, and vendors. Updates staff on changes to policies and procedures as directed by CCO. Manage staffing needs with contracted agencies. Physician Certification F2F, and Recertification and Compliance monitoring. Maintains up-to-date patient records Provides support to families at the time of the patient's death with coordination of services. Works with pharmacy, therapists, DME suppliers, and other health care providers in the absence of the case manager to ensure coordination of patient care. Remains knowledgeable about Federal Medicare, state licensure requirement and Joint Commission standards to ensure compliance with regulations that govern admissions and Hospice care. Demonstrates good rapport and cooperative working relationships with staff, patients, physicians, and community agencies.
