About The Position

Applying the principles of Systems of Care, the Supervisor of OhioRISE Care Coordination is responsible for developing and overseeing a team of Care Coordinators who ensure and cultivate flexible, family-focused, community-based responsive services based on the High-Fidelity Wrap Around Model of care coordination for Tier II Moderate clients and Tier III Intensive clients in alignment with The Wingspan Care Groups Mission, Vision and Core Values.

Requirements

  • May be licensed or an unlicensed practitioner in accordance with rule 5160-27-01 of the Administrative Code.
  • Bachelor’s degree with 2 years experience and license, or Master’s degree or higher with 1 year of experience, in children's behavioral health, child welfare, developmental disabilities, juvenile justice, or a related public sector human services or behavioral health care field, providing community-based services to children and youth, and their family or caregivers.
  • Have a background and experience in one or more of the following areas of expertise: family systems, community systems and resources, case management, child and family counseling or therapy, child protection, or child development.
  • Valid Ohio Driver’s License and maintains a driving record that allows that individual to be insurable with the insurance company providing Wingspan Care Group with vehicle insurance.
  • Ability to visit clients’ homes (may or may not be handicapped accessible).
  • Excellent verbal and written communication skills; strong teamwork and organization/time management skills.

Responsibilities

  • Participate in the recruitment, interviewing and training of potential new Care Coordinators.
  • Ensure achievement of monthly and yearly goals for staff and program.
  • Provide both formal and informal training for assigned staff; training should be individualized, specialized and ongoing.
  • Conduct performance evaluations and develop action plans for assigned staff for continued professional development. Develop corrective action plans as needed.
  • Provide regular supervision with assigned staff to ensure open lines of communication - provide both verbal and written feedback.
  • Facilitate meetings with staff, at least on a bi-weekly basis or as defined by regulatory expectations, and other departmental meetings to ensure quality, holistic treatment of families and consistency in program planning.
  • Reviews program specific paperwork to ensure ongoing quality with documentation and around treatment planning.
  • Ensures development of an appropriate aftercare plan regarding ongoing client needs.
  • Oversee the development and implementation of all treatment and service plans for clients and their families.
  • Ensure that staff is trained in the developmental/treatment strategies outlined in the treatment plans and utilizing approved program strategies when working with clients and their families.
  • Participate in required fidelity reviews as coordinated by Case Western’s Child and Adolescent Behavioral Health Center of Excellence.
