The primary function of the social work supervisor is to oversee and direct supervision of the Social Workers in Population Health Department. The social work supervisor will lead a team, mentor, develop and implement training. The social work supervisor provides social work services and discharge planning to patient populations across the continuum of illness. The Social Work Supervisor will provide direct care of complex patients within the hospital and referred to the Wagner Wellness Van. This role will act as a liaison for the hospital with multiple outside resources.

  • Lead a team, mentor, develop and implement training.
  • Provide social work services and discharge planning to patient populations across the continuum of illness.
  • Provide direct care of complex patients within the hospital and referred to the Wagner Wellness Van.
  • Act as a liaison for the hospital with multiple outside resources.
  • Graduate of a master's degree program in social work from a college or university approved by the Council of Social Work.
  • LCSW - Licensure as a Social Worker in the state of MD required.
  • Certification is required.
  • Minimum of 3-5 years previous experience in social work or related capacity preferably in medical/health setting.
  • Minimum of 2-3 years previous supervisory experience.
  • Full-time exempt, Monday-Friday.
  • The ability to round on supervised staff during PM shifts required on a monthly basis (average 6-8 hours per month).
  • Must be available to staff after hours by phone to handle crises.
