About The Position

The Clinical Supervisor of Home Health is a Registered Nurse (RN) who works under the supervision of the Executive Director to direct the total management of clinical operations in the branch office with assigned staff. This role is responsible for the overall direction, quality, and efficiency of managing home health services. The Clinical Supervisor demonstrates knowledge of policies and procedures and time management. They establish, implement, and evaluate goals and objectives for home health services that meet and promote the standards of quality and contribute to the total organization. The RN participates in patient, caregiver, and family counseling and education, communicates with all physicians involved in the plan of care, and provides services ordered by the physician as indicated in the plan of care. The RN will also participate in the development and revision of the plan of care in partnership with the patient, patient representative, and caregiver(s), and may administer skilled care to patients, performing field visits when required.

Requirements

  • Active Registered Nurse (RN) license in the state(s) of practice.
  • CPR certified with American Heart Association or American Red Cross.
  • Extensive nursing experience, preferably with an emphasis in Home Health.
  • Minimum of 3-5 years of recent clinical leadership or supervisor experience in home health, home care, or hospice.
  • Proficiency with EMR systems (e.g., WellSky/Kinnser).
  • Clinical management level experience with the ability to supervise, direct, and mentor personnel.
  • Experience in staff management, mentoring, and team supervision.
  • Knowledge of Medicare, accreditation criteria, and standards.
  • Solid understanding of Medicare regulations, OASIS documentation, and accreditation standards.
  • Current driver's license, auto liability insurance, and reliable transportation.

Responsibilities

  • Direct the total management of clinical operations in the branch office.

  • Participate in patient, caregiver, and family counseling and education.
  • Communicate with physicians and other health care practitioners regarding the plan of care.
  • Administer services ordered by the physician as indicated in the plan of care.
  • Participate in the development and revision of the plan of care.
  • Perform field visits as required.
  • Contribute to all phases of the QAPI program.
  • Participate in ongoing interdisciplinary assessments of patients.
