About The Position

Responsible for supporting inpatient and outpatient pharmacy services. Review and interpret prescription orders and perform drug utilization reviews to identify potential interactions or inaccuracies. Ensure accurate dispensing of medications, oversee work of support staff, providing pharmaceutical information to other healthcare professionals, monitor drug therapies, and provide pharmaceutical expertise to patients on the safe use of medications. The role will also be responsible for interdisciplinary rounds and comprehensive medication reviews of all clients admitted to our inpatient units.

Requirements

  • Licensed Pharmacist in Colorado required
  • More than five years of experience as a pharmacist or completion of one year of a clinical pharmacy residency program required

Nice To Haves

  • Doctor of Pharmacy degree preferred
  • Experience in an inpatient pharmacy role preferred
  • Experience working in behavioral health preferred

Responsibilities

  • Prepare medications by reviewing and interpreting physician orders and completing a drug utilization review for each medication, ensuring medication is safe and appropriate for each patient based on indication and patient-specific factors.
  • Helps with outpatient operations including data verification, filing of medications and point of sale medication dispensing.
  • Ensure efficient and proper dispensing of medications. Verify correct patient, correct medication, correct dose, correct directions, and correct quantity. Verification of all medication orders during inpatient stay.
  • Assure inventory levels are maintained at their most efficient and effective levels.
  • Work with providers to create and update patient care plans during rounds and as needed, ensuring appropriate medication use, including MAT (Medication Assisted Treatment), acute psychiatric meds and withdrawal treatment medications.
  • Conduct quality assurance monitoring and auditing to demonstrate compliance with inpatient and department policy and procedures, regulatory, and accreditation standards.
  • Completes comprehensive medication review for clients being admitted to one of the inpatient units. Counsel patients on medications at discharge and during stay, providing information on drug interactions, side effects, and dosage and storage of medications.
  • Assess quality of services delivered and coordinates client care services with medical and clinical staff, and other programs/departments as needed.
  • Comply with state and federal drug laws as regulated by the State Board of Pharmacy, the Drug Enforcement Administration, and the Food and Drug Administration by maintaining records of controlled substances, removing outdated and damaged drugs from the pharmacy inventory, supervising the work results of support personnel, maintaining current registration, studying existing and new legislation, anticipating legislation and advising management on needed actions. Maintain established procedures concerning quality assurance, security of controlled substances, and disposal of hazardous waste drugs.
