About The Position

The Transitions of Care Clinical Staff Pharmacist is critical in ensuring patients are prepared to be transitioned to different levels of care and have the information necessary to take their medications safely, appropriately, and consistently. The pharmacist will review the medications prescribed for appropriate dosing, frequency, and indications and address any discrepancies or recommendations with the patient's primary team or prescriber. The pharmacist will provide counseling on their medication regimen and appropriate resources to educate the patient about medication instructions, uses, side effects, and additional medication therapy management considerations. The pharmacist may also participate in specialty consults such as anticoagulation counseling, heart failure counseling, outpatient parenteral antimicrobial therapy and home infusion review, solid organ transplant and left ventricular assist device (LVAD) education and pill box fill, and discharge counseling for high-risk patients discharging from UT Southwestern Clements University Hospital or Zale-Lipshy Rehab facility. There may be additional opportunities that arise as the program develops.

Requirements

  • Doctor of Pharmacy (PharmD) degree or equivalent from an American College of Pharmaceutical Education (ACPE) accredited school of pharmacy required.
  • Good general health and stress-coping ability.

  • Licensed Pharmacist by Texas State Board of Pharmacy (TSBP) Upon Hire
  • (BLS) BASIC LIFE SUPPORT certification with a course accredited by the American Heart Association (AHA) Upon Hire

Nice To Haves

  • Completion of a PGY-1 Pharmacy Practice Residency from an ASHP-accredited residency program preferred.
  • 2 years or more of clinical pharmacist experience

Responsibilities

  • Performs job responsibilities per organizational and departmental policies, procedures, performance improvement objectives, environmental/infection control standards, and state and federal laws and regulations.
  • Performs all computer order entries accurately, thoroughly, and timely. Reviews/updates medication profiles (therapies, allergies, height, weight, etc.) and documents recommendations appropriately.
  • Reviews medication orders, prescriptions, and home infusions for therapeutic appropriateness, duplications, interactions, overall effectiveness, and safety. Seeks clarification if necessary and documents interventions or recommendations (i.e., ADR).
  • Consistently works (multi-tasks) and solves problems independently and effectively with minimal supervision. Takes charge when necessary and is looked to for direction. Makes the right decisions and uses good judgment. Is cross-trained and multifunctional, maintaining competency in multiple areas.
  • Participates in cost-effective use and selection of pharmaceuticals.
  • Communicate drug information (verbally and in writing) to physicians, nurses, etc., courteously and professionally. Assists in patient/family education and counseling regarding drug therapy and side effects. Also, effectively documents clinical activities, demonstrating cost savings and concisely conveying critical information required for follow-up by fellow pharmacists. Strives to improve patient satisfaction, as measured by Press Ganey scores.
  • Assists in supporting pharmacy technicians (ie med history technicians) with any clinical questions and/or guidance that may be needed to complete their tasks.
  • Identifies opportunities for improvement in the patient transitions of care process.
  • Actively enhances personal growth and development by participating in educational programs, in-services, and meetings. Actively participates in new employee orientation and training. Willingly takes on additional projects as needs develop.
  • Appropriately demonstrates openness to variety within the work environment and consistently respectfully deals with all customers. Supports teamwork and cooperation with co-workers in daily activities. Readily volunteers assistance to help co-workers complete tasks.
  • Actively participates in the clinical program. Supports P&T initiatives by reviewing patients on high-risk medications, Collects applicable data/measures and communicates opportunities for system or team improvement opportunities. Participate on committees or workgroups as assigned.
  • Communicates effectively with patients regarding high-risk medications and provides a comprehensive medication management review for patients at high risk for re-admission.
  • Provides educational materials and in-service for medical, nursing, and pharmacy team members.
  • Assists with Joint Commission standards and pharmacy safety initiatives.
