About The Position

Clark Regional Medical Center is seeking a dedicated and experienced Clinical Staff Pharmacist to join our pharmacy team. This role is responsible for delivering high-quality pharmaceutical care to hospitalized patients, ensuring safe, effective, and cost-conscious medication use, and supporting clinical and operational excellence across the department. The pharmacist will also serve as a pharmaceutical expert and resource for clinical staff, promoting best practices in medication therapy and patient safety. Collaborate with physicians and healthcare providers to improve medication therapy outcomes. Participate in medication reconciliation to ensure accurate patient medication histories. Provide pharmacokinetic assessments and therapeutic drug monitoring. Attend interdisciplinary team rounds and meetings to support collaborative care planning. Promote safe, appropriate, and cost-effective medication use. Pharmacy Operations: Review and verify medication orders for accuracy and appropriateness. Perform the final verification of medications prior to dispensing to patients and loading them into automated dispensing cabinets. Supervise pharmacy support personnel to ensure efficient and coordinated services. Ensure compliance with state and federal pharmacy laws and safety regulations. Monitor drug usage to meet internal and external quality standards. Maintain proficiency in pharmacy-related computer systems. Additional Duties: Provide staffing support for the anticoagulation clinic, including patient education, monitoring, and dose adjustments. Perform other duties as assigned to support departmental and hospital goals. Minimum Qualifications: Education: Bachelor’s Degree Required; Doctoral Degree Preferred Licenses: Pharmacy License in the state of Kentucky Hospital pharmacy experience preferred. Residency training desired. Requirements

  • Bachelor’s Degree Required
  • Pharmacy License in the state of Kentucky
