About The Position

Assure the appropriate selection, dosing, administering, and dispensing of medication orders in specialized areas, such as nuclear medicine, oncology, cardiology. Serve as a subject matter expert in pharmacy discipline and provide guidance across pharmacies and to clinical leadership. Review and assess medication orders to identify, prevent and resolve potential and actual drug related problems. Recommend formulary alternatives when appropriate. Compound and dispense medications for accurate delivery to patients. Provide drug information and education to medical staff and patients. Communicate with physicians and other clinicians as necessary to facilitate care. Oversee technical personnel who assist in the preparation, maintenance, and dispensing of medications and supplies. Assist with academic functions such as serving as a preceptor for pharmacy students. May float to provide coverage of pharmacy needs. Provide patient counseling when required. This PRN pharmacist position will support the Pharmacotherapy Clinic at Saint Joseph Hospital - Chicago. The Pharmacotherapy Clinic specializes in chronic disease state management, anticoagulation management and post-discharge counseling. Coverage may be in-person or remote. No evening, weekend or holiday coverage required.

Requirements

  • Pharmacist credentialed from the Illinois Department of Financial and Professional Regulation obtained prior to hire date or job transfer date required.
  • Bachelor's or advanced degree in Pharmacy required.
  • Active licensure in good standing with the Illinois Department of Financial and Professional Regulation (IDFPR)
  • Completion of an American Society of Health System Pharmacists (ASHP) accredited PGY-1 Residency and ≥ 1 years of experience in ambulatory care practice OR Completion of an ASHP accredited PGY-2 Residency in one of the following specialties: Ambulatory Care, Pharmacotherapy, Family Medicine, Cardiology, Internal Medicine, Geriatric, Nephrology, or Transitions of Care OR Doctorate of Pharmacy (PharmD) or Bachelor of Pharmacy (B.S. Pharm) with at least five years of ambulatory care experience.

Nice To Haves

  • Clinical pharmacists are preferred to hold nationally recognized certifications (e.g. BCACP, BCPS, BC-ADM, CDCES, etc)
