About The Position

The Licensed Clinical Social Worker will report to the Associate Director of Operations. They will demonstrate support of the Company’s values by acting as an advocate to help clients gain access to resources and assist them with managing personal and family issues. They must provide mental health/psychological management to clients and families to help in coordination of care and the management of a patient’s individual needs through and beyond the course of treatment. The Licensed Clinical Social Worker must possess the ability to assess, coordinate, plan, monitor, implement and evaluate the patient’s mental health plan of care while ensuring and facilitating the achievement of quality clinical outcomes. The Licensed Clinical Social Worker will support the mission of the Company and the Care Management department.

Requirements

  • Minimum of Master of Social Work.
  • Minimum of 3 years of experience in the field of social work and at least 1 year in oncology preferred.
  • Maintain license as Licensed Clinical Social Worker (LCSW)

Nice To Haves

  • Minimum of 1 year in oncology preferred.

Responsibilities

  • Maintain knowledge of up-to-date social work trends
  • Provide basic education to all new patients.
  • Utilize and provide resources to answer patient questions.
  • Educate patient about visit expectations.
  • Effectively and efficiently triages patient psychosocial issues.
  • Utilize interventions appropriately and communicate with the physician/clinic staff as necessary.
  • Obtain needed community resources utilizing the tools provided by management
  • Follow patients throughout the care continuum and act as a liaison for the patient and caregiver.
  • Educates patients and families about psychosocial services.
  • Assess the patient’s mental and emotional status and provide appropriate interventions.
  • Documents timely and thoroughly in the electronic medical record.
  • Complies with CMS regulations and follows Care Management Standard Operating Procedures.
  • Keeps Director promptly and fully informed of any problems or unusual matters of significance.
  • Maintain and ensure the confidentiality of all patient and employee information at all times.
  • Ensure adherence to legal and company policies and procedures and undertake disciplinary actions if the need arises
  • Appropriately document patient communications and visits in OncoEMR.
  • Go through patient EMR chart to obtain demographic information needed prior to first patient contact while maintaining HIPPA guidelines
  • Be able to identify the appropriate actions for a patient based on EMR documentation
  • Utilize HIPAA standards to protect the privacy of confidential counselling session documentation.
