Position Summary: Responsible for the provision of social work to patients in need of these services throughout the cancer center. Works in active partnership with the MD’s, NP’s and RN’s and other disciplines to develop and implement a well-coordinated personalized plan to address patient needs across the continuum. The social worker assesses plans and facilitates options and services to meet an individual’s unique and complex needs. This is accomplished by addressing both the individual patient’s medical and psychosocial needs, as well as the resources available. There is a continuing commitment to maximizing the quality of life for each patient and enhancing community health. The Social Worker performs duties and responsibilities in a manner consistent with the hospital mission, values and NASW Code of Ethics. Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the patients served. Responsible for promoting teamwork with all members of the healthcare team. Performs all duties in a manner consistent with St. Luke’s mission and values.

Requirements

  • Master’s Degree in Social Work
  • Licensed Clinical Social Work status or eligibility for provisional license required with attainment of LCSW in process
