Requirements

  • Must possess a high school diploma or GED
  • Must possess a degree in: Social Work
  • MSW with 2 years post graduate experience in health care setting
  • BSW with 10+ years experience in health care setting
  • Minimum of LSWA, LGSW or LCSW. LCSW-C preferred
  • Age Specific Care Provided For: Neo-natal Pediatrics Adolescents Adults Geriatrics
  • Computer proficiency and technical aptitude with the ability to utilize one or more of the following: Microsoft Word, Power Point, Excel Microsoft Outlook Meditech (if applicable to position) Epic (if applicable to position) Performance Manager/NetLearning
  • Must be able to read, write, and speak the English language in an understandable manner.
  • Must be able to work independently and possess the ability to make independent decisions when circumstances warrant such action.
  • Must possess the ability to effectively communicate and deal tactfully with other personnel, visitors, patients, physicians, customers, and the general public.
  • Must be flexible, have personal integrity, and the ability to function effectively as a team with other employees.
  • Must be able to follow written and oral instructions.
  • Must possess skills in: Accurate verbal and written expression in the English language Work organization, problem solving Psycho-social counseling Conflict resolution Identification of crisis situations er
  • Must be able to perform: Bio-psychosocial assessments Crisis counseling Community resource referrals Documentation
  • MD drivers license and use of car during the working hours may be required for agency visits and professional meetings.

Nice To Haves

  • Fluency in Spanish preferred.

Responsibilities

  • Provides discharge planning and counseling to patients of all ages through development and implementation of an individual plan of care which includes assessment and resource linkage based on needs of patient and family.
