About The Position

Supports the mission of People's Community Clinic by providing therapeutic behavioral intervention and social services to high-risk patients and interfacing with other health and human service provider agencies and individuals to promote patient care and community support. This position works as a member of an integrated care team to coordinate patient care and support patients and families in self-management, self-efficacy, and behavior change. Care team members are expected to assess and manage the mental health needs of the patient population and to communicate effectively with all patients, especially vulnerable populations.

Requirements

  • Master's Degree in Social Work.
  • Prefer a minimum of two years' experience as a social worker and/or therapist.
  • Bilingual (English/Spanish) required.
  • Ability to conduct a thorough psychosocial assessment, formulate a diagnosis, and make and/or implement treatment recommendations.
  • Crisis intervention and case management skills.
  • Familiarity with psychotherapeutic theories and strategies.
  • Knowledge and skill in individual and family therapy.
  • Strong written and verbal skills.
  • Record keeping and principles of patient education.
  • Group facilitation and social work practice.
  • Strong interpersonal skills and knowledge of community resources.
  • Program development and teaching/mentoring abilities.
  • Creating and maintaining referral linkages.
  • Ability to work effectively and cooperatively as a member of a team and with other clinic staff.
  • Ability to communicate effectively with persons of varying ages and cultural and socioeconomic backgrounds.
  • Effective networking skills with local and area resources and agencies.
  • Flexibility, initiative, dependability, leadership, and time management.

Responsibilities

  • Provide direct service including therapeutic intervention and social services for high-risk patients.
  • Conduct screenings for the Integrated Behavioral Health Program.
  • Complete intake assessments, including client psychosocial history, current needs, diagnosis, and treatment recommendations.
  • Provide short-term, individual, and family therapy to People's clients.
  • Provide crisis intervention to patients as needed.
  • Provide information and referral for People's clients to other service providers (social, financial, and mental health, etc.).
  • Maintain detailed current referral information.
  • Document services provided to clients per guidelines and policies.
  • Provide follow-up to patients to assess suitability of and follow through with referrals.
  • Document referrals and rates of client follow-through.
  • Identify opportunities to improve workflow, work processes, and patient care, and work cooperatively to implement these and other quality improvements.
  • Adhere to HIPAA guidelines.
  • Communicate regularly with other service providers and referral sources to assure current information and to facilitate client access.
  • Represent People's at appropriate interagency forums and meetings.
  • Provide referral and information updates to other People's staff (medical, health education, and admissions) as needed.
  • Develop professional relationships with referral sources.
  • Develop and implement outreach strategies as needed.
  • Attend all staff and team meetings.
  • Work cooperatively with departmental supervisor, team members, and general staff to ensure service standards and to identify and resolve problems.
  • Participate in program evaluation and program planning in the area of responsibility.
  • Provide leadership and expertise to the clinic staff in areas of mental health issues, therapeutic intervention strategies, referral methods, and other social work issues.
  • Adhere to Clinic policies and procedures.
  • Promote professionalism and ethical, client-centered interaction within the Clinic.
  • Maintain current knowledge of issues related to mental health diagnosis and treatment; social work skills and practices; social service and public health issues related to clients receiving services.
  • Supervise student social work interns.
  • Provide instruction as needed to interns, residents, and nursing students on social work and mental health topics.
