About the position

Cone Health Social Workers provide resources, discharge planning, and allocation in consultation with the hospital multidisciplinary team. The inpatient care management team is a focal point of care delivery and is well-received and valued by the organization for our role in ensuring patients have a smooth transition of care back to the communities we serve. This is an exciting time in healthcare, and we are seeking to recruit those who want to be a part of the journey that will make a positive difference in the lives of many individuals. Responsibilities

  • Provide social work services to patients and their families to assist in meeting psychosocial and clinical needs.
  • Identify and serve the needs of patients, both inside and outside the clinical setting utilizing therapeutic and other clinical techniques.
  • Determine which patients to focus clinical social work efforts on to help staff and patients alike be in the correct environment with appropriate social work support.
  • Analyze information from both the patient and clinical staff to understand the patient's situation and communicate this information to all parties.
  • Link patients with community resources and community health opportunities based on clinical knowledge and experience.
  • Craft a care plan for patients with minimal assistance from clinical staff.
  • Educate staff, patients, and families about critical information related to care.
  • Provide crisis support and leadership in high-tension situations for staff, family, and patients.
  • Complete psychosocial assessments for patients to understand their situation.
  • Utilize various counseling and therapy techniques to help place the patient and their family at ease.
  • Perform other duties as assigned.

Requirements

  • Masters of Social Work.
  • Minimum of two (2) years of recent experience in a health-related environment.
  • Licensed Clinical Social Worker.
