Your expertise is valued, your voice is heard, and your work makes a real difference every day. We are seeking a full-time, 40-hour Clinical Social Worker to join the “Marathon" team. The entire team supports the following territories: Allston Belmont Brighton Brookline Chestnut Hill Newton Corner Newtonville Watertown Enjoy the freedom to design your own schedule while delivering 1:1 care - because your work-life balance matters. The desired schedule is Monday through Friday. Anticipated daily volume is 4-5 visits per day plus chart review, visit preparation, documentation, and care coordination. This position is responsible for performing social work assessments and interventions for home care with the occasional request to see a home hospital patient. They are dedicated to patient and family centered care that values personal self-determination, skilled at managing multiple priorities at once, and engaging in creative, compassionate and ethical problem-solving. As a member of a multidisciplinary health team, they perform comprehensive assessments and develop treatment plans that integrate the medical, social and resource issues that impact individual patients, families and at-risk populations. They work collaboratively with all healthcare team members to develop and implement treatment plans that support patient-centered plan of care for both individual patients and the medical community.
