About The Position

The Clinical Social Worker at Duke Primary Care will provide psychosocial assessments, diagnosis, treatment, and discharge planning for patients and families, focusing on emotional, mental, and substance abuse disorders. This role includes functioning as a team leader and providing comprehensive psychosocial treatment plans, crisis intervention, and training for healthcare professionals. The position covers both pediatric and internal medicine at Sage Road, contributing to a collaborative healthcare environment.

Requirements

  • Master's degree in social work from an accredited school of social work.
  • Current licensure as a licensed clinical social worker (LCSW) by the NC Social Work Certification and Licensure Board.
  • Two years of post-internship social work experience in a healthcare setting or social service agency for Level I.
  • Five years of post-master's experience in a healthcare or social service agency for Level II, with experience in leadership and supervision.

Nice To Haves

  • Experience in a clinical setting preferred for Level II.
  • Strong supervisory and leadership skills for Level II.

Responsibilities

  • Provide psychosocial assessments of patients and families to identify emotional, social, and environmental strengths and problems.
  • Formulate, develop, and implement comprehensive psychosocial treatment plans.
  • Conduct crisis intervention and various therapy sessions including individual, marital, family, and group therapies.
  • Screen, identify, diagnose, and manage mental health and substance abuse problems.
  • Provide training to healthcare professionals on psychosocial issues.
  • Assist with the management and treatment of victims of abuse and neglect.
  • Provide consultative services to healthcare team members as needed.
  • Educate patients and families on adaptation to diagnoses and treatment.
  • Participate in multi-disciplinary healthcare teams and represent clinical social work perspectives.
  • Contribute to academic activities such as committees, conferences, and research.
  • Maintain knowledge of community resources and coordinate patient needs pre and post hospitalization.
  • Document assessments and interventions according to guidelines and standards.
