About The Position

The Clinical Social Worker in Dialysis at the University of Virginia provides essential psychosocial support to patients and their families dealing with end-stage renal disease (ESRD). Requirements

• Masters degree in Social Work from a CSWE accredited social work program required.
• No prior experience required.

Responsibilities

• Provide complete bio-psycho social assessments at regular intervals based on patient presentation and CMS regulations.
• Fully participate in the care planning process and facilitate patient engagement in the Plan Of Care.
• Deliver direct clinical services to patients and their families based on individual needs, preferences, and resources.
• Provide case management services to maximize eligibility for entitlements, community resources, and benefits programs.
• Document all activities relevant to assessment, direct clinical work, casework treatment planning, and crisis intervention according to UVA MC Renal Services Social Work policy and legal guidelines.
• Maintain knowledge of resources and changes in laws or guidelines relevant to Clinical Social Work practice in Virginia and CMS.
• Participate in professional development and maintain relevant licenses, certifications, and continuing education requirements.
• Assess own practice and seek input for improvement, participating in training for skill enhancement.
