About The Position

Involves crisis intervention, psychosocial patient assessments and interventions, mandated reporting, information and referral, discharge planning, and coordination of health and mental health services with community providers. Works in concert with physicians, nurses, utilization management, discharge planners, and other allied health professionals; may be member of a structured case management team. Assists patients and their families with addressing and resolving the social, financial, and psychological problems related to their health condition and facilitates the patient's movement through the health continuum. The social worker works as part as a dyad with the RNCC and MD to help with discharge planning for all inpatient units (med surg, tele, ICU) and the emergency department. They usually carry a census of 20-30 patients. Requirements

  • REQUIRED FOR CLINICAL SOCIAL WORKER II This position requires flexibility to orient and work at all UCSF Medical Center locations.
  • Master’s degree from a CSWE accredited school of Social Work.
  • A minimum of three years' experience in a supporting individual with complex health conditions.
  • Knowledge and abilities essential to the successful performance of the duties assigned to the position.
  • Demonstrated clinical experience utilizing skills in individual, family and group interventions.
  • Knowledge of issues of access to health care and disability benefits.
  • Demonstrates resourcefulness, effective written and oral communication, diplomacy, organizational and analytic skills.
  • Self-directed, assertive and creative in problem solving, systems planning and patient care management in a high volume and complex work environment.
  • Basic computer skills.
  • Ability to work effectively and collaboratively with interdisciplinary teams.
  • ASW REQUIRED FOR CLINICAL SOCIAL WORKER III All items from Clinical Social Worker II Requirements, as well as items below Licensed in Clinical Social Work in the state of California/Board of Behavioral Sciences.
  • Active LCSW License

Nice To Haves

  • The CSW III has advanced clinical knowledge and skills and is distinguished from the CSW II by leadership activities including participation in advanced program planning, provision of educational programs, clinical supervision, consultation to the clinical team, other departments, and the community.
  • Knowledge of community resources critical to supporting patients, their caregivers and families
  • Preferably worked with vulnerable populations.
  • Therapeutic presence (i.e.: reflective listening, conveyance of empathy), experience carrying out mental health crisis management, and in explaining complicated systematic and interpersonal concepts in Spanish, Cantonese, or Russian
  • Applicants with a clear knowledge of Black, Indigenous, and People of Color cultural groups; including their strengths, needs, and communities
