The Wound, Ostomy, and Continence Clinical Services Nurse (WOC CSN) provide leadership in the care of patients with stomas, draining wounds, or incontinence, throughout the organization. This includes the utilization of clinical expertise to provide preoperative and postoperative care, counseling, and teaching to patients 0-99 years of age in need of wound, ostomy, and/or continence management. S/he has primary responsibility for educating nurses to ensure knowledge and competency in basic wound, ostomy, and continence management; and works collaboratively with the interdisciplinary team in a variety of specialties in planning and coordinating the care.

Requirements

  • Graduation from an accredited Bachelors Degree in Nursing program.
  • 2 years of appropriate clinical experience required.
  • Current or compact RN licensure in the state of North Carolina.
  • BLS required.
  • Nationally recognized certification in Wound and Ostomy management is required: CWOCN®, CWCN®, COCN®, or CWON®.

Nice To Haves

  • Strong preference for CWON or CWOCN certification.
  • Ability to independently seek out resources and work collaboratively.
  • Knowledge of professional theory, practice and procedure.

Responsibilities

  • Independently plan and provide professional nursing care for patients in accordance with physician orders and established policies and procedures.
  • Identify patient caseload through referrals and active case finding.
  • Conduct complete and accurate patient assessment.
  • Provide specialized nursing treatment, care and education to patients with wounds.
  • Provide specialized nursing treatment, care, stomal siting, and education to patients undergoing urinary or stool diversion.
  • Provide specialized nursing treatment, care, and education to patients with draining wounds or requiring continence management.
  • Select, develop and/or implement evidence-based clinical interventions and systems to achieve defined patient and system outcomes.
  • Serve as a resource to patients and families, providing preoperative counseling and follow-up problem solving after discharge from the hospital.
  • Utilize comprehensive assessment skills in the evaluation of stoma and wound care products and supplies.
  • Assist patients and families with the understanding of their diagnosis and the management thereof.
  • Identify patient/family education needs and priorities.
  • Coach patients and families to help them navigate the healthcare system.
  • Utilize teaching and learning theories to assist patients and families with physical and emotional impact of body changes and chronic illness.
  • Participate in developing, implementing and evaluating patient education resources and programs.
  • Assess learning needs of nursing staff; participate in planning and implementing appropriate educational forums.
  • Establish collaborative relationships within and across departments that promote patient safety, culturally competent care, and clinical excellence.
  • Serve as a resource and consultant to nursing members of the healthcare community.
  • Participate in and support accreditation and regulatory activities of Duke Hospital.
  • Facilitate the provision of clinically competent care by staff/team through education, role modeling, teambuilding, and quality monitoring.
  • Promote evidence-based practice through participation in developing, implementing, and evaluating process standards.
